신생아 갑상선 기능 항진증, Neonatal thyrotoxicosis

- 신생아의 갑상선 기능이 항진되고 그로 인해 갑상선 기능 항진증을 나타내는 병을 신생아 갑상선 기능 항진증이라고 한다.
- 임신 중 임신부에게 그레이브스 병 등이나 그 외 갑상선 기능 항진증이 있지만 그런 병이 있는지 모르고 임신 했던 임산부에게 태어난 신생아 또는 잘 치료되지 않은 갑상선 기능 항진증이 있는 임산부에게 태어난 신생아에게 갑상선 기능 항진증이 생길 수 있다.
신생아 갑상선 기능 항진증의 증상 징후
- 생후 24시간 이내 신경이 예민하고 신체를 과도히 움직이고 손발이 떨리고 얼굴에 홍조가 생기고 비정상적으로 땀이 많이 나고 과도히 많이 먹지만 체중이 증가되지 않고 안구 돌출이 생기고 빈맥증이 생길 수 있고 갑상선이 비대 되어 있을 수 있다.
- 이런 증상 징후를 적절히 치료하지 않으면 생후 3~6 개월 동안 지속되다가 대개 증상 징후가 자연히 없어진다.
신생아 갑상선 기능 항진증의 진단
- 임신 중 임산부가 갑상선 기능 항진증이 있었다는 사실, 증상 징후, 진찰소견, 테트라요오드티로닌(Tetraiodothyronine/Thyroxine/T4), 삼옥소타이로닌(Triiodothyronine/T3), 갑상선 자극 호르몬 (Thyroid-stimulating hormone/TSH), 장시간 작동성 갑상선 자극기(Long-acting thyroid stimulator/LATS) 등 검사로 진단한다.
신생아 갑상선 기능 항진증의 치료
- 원인에 따라 치료한다.
- Iodine, Propylthiouracil, 또는 Methimazole 등으로 치료한다.
- 필요에 따라 산소호흡 치료를 한다.
