아이소밸러르 산혈증, Isovaleric acidemia

  • 아이소밸러르산 CoA 디하이드로지네이스 결핍(Isovaleryl CoA Dehydronenase Deficiency)으로 생기는 선천성 대사 질환의 일종이다.
  • 상염색체 약성으로 유전된다. 
  • 심한 키토산혈증, 혈소판 감소증, 호중구 감소증, 저칼슘증, 고암모니아증, 고혈당증 등이 이 병으로 생길 수도 있고 때로는 생기지 않는다. 
  • 급성으로 생길 때는 심한 구토, 산혈증, 기민상태, 경련, 혼수가 생길 수 있고, 사망할 수도 있다. 
  • 특이한 단 맛 냄새가 발에서 나기도 한다. 
  • 이 병의 증상 징후가 출생 후 며칠 내에 나타나는 급성형도 있고 생후 몇 개월 내지 몇 년 사이에 발생되는 만성형도 있다. 
  • 증상 징후가 경미하게 나타나기도 하고 아주 심하게 나타나기도 한다. 

아이소밸러르 산혈증의 진단

  • 병력, 증상 징후, 진찰, 혈액 소변검사와 혈중 아이소길 초산 혈중농 치 증가를 종합해서 진단한다. 
  • 요즘 미 코네티컷 주에서는 모든 신생아들의 피로 선천성 신진대사 이상의 유무를 진단할 때 아이밸러르산 혈중농도 검사도 한다. 

아이소밸러르 산혈증의 치료

  • 급성으로 이 병이 생기느냐 또는 만성으로 이 병이 생기느냐에 따라 치료를 달리한다.
  • 대증치료를 하고
  • 저단백질 식이요법과 글리세린(Glycerin)과 카르니틴(Carnitine) 추가 공급으로 치료한다.
  • 필요에 따라 교환수혈 치료, 복막투석 치료 등으로 적절히 치료하면 정상적으로 발육할 수 있다.

Isovaleric acidemia

• It is a type of congenital metabolic disease caused by Isovaleryl CoA Dehydronenase Deficiency.

• Inherited as an autosomal weakness.

• Severe chitosanemia, thrombocytopenia, neutropenia, hypocalcemia, hyperammonosis, and hyperglycemia may or may not occur with this disease.

• In acute cases, severe vomiting, acidemia, drowsiness, convulsions, coma, and death may occur.

• Your feet may have a peculiar sweet smell.

• There is an acute form, in which the symptoms and signs of the disease appear within a few days after birth, and the chronic form, which develops between months and years after birth. • Symptoms can be mild or very severe.

Diagnosis of isovaleric acidemia

• Diagnosis is made by combining medical history, symptomatic signs, examination, blood urinalysis, and elevated blood isogilacetate levels.

• Nowadays, in Connecticut, the state of all newborns is tested for blood levels of ivaleric acid when diagnosing the presence or absence of congenital metabolic abnormalities with fatigue.

Treatment of isovaleric acidemia

• Treatment differs depending on whether the disease develops acutely or chronically.

• Treat symptomatically

• Treat with a low-protein diet and supplemental supplementation of glycerin and carnitine. • If necessary, proper treatment with exchange transfusion treatment or peritoneal dialysis treatment can lead to normal development.
