원발성 알도스테론증, Primary aldosteronism

원인 cause	이 병을 콘 증후군 Conn’s syndrome,이라고도 한다. 부신 피질에서 알도스테론 과잉 생성으로 발생하는 질환이다.. 원발성 알도스테론증의 원인은 부신 과형성, 부신 선종, 악성 종양 또는 부신 가족성 알도스테론증 등으로 생길 수 있다. • This disease is also called Conn’s syndrome. • It is a disease caused by excessive production of aldosterone in the adrenal cortex. • Primary aldosteronism can be caused by adrenal hyperplasia, adrenal adenoma, malignancy, or • It may be caused by adrenal familial aldosteronism.
증상 symptoms	알도스테론 호르몬이 과잉 생성되면 체내 나트륨 증가하고 칼륨이 저하된다. 고혈압이 발생하고 심근 경색증, 심장 비대나 심부전 소변으로 칼륨의 배설이 증가하여 저칼륨혈증, 손발 저림, 근육 경련, 근무력증 등의 증상이 생길 수 있다. 시력 장애, 두통 다뇨, 신장 부전증, 불규칙 박동 등이 생길 수 있다. • When the aldosterone hormone is overproduced • Increased sodium and decreased potassium in the body. • High blood pressure develops and • Myocardial infarction, enlarged heart or heart failure • Hypokalemia due to increased excretion of potassium in the urine; • Symptoms such as numbness in the limbs, muscle cramps, and myasthenia gravis may occur. • Vision impairment, headache • polyuria, renal insufficiency; • Irregular beats may occur.
진단 diagnosis	병력 + 증상+진찰소견 + Aldosterone과 Renin 검사 등 그 외 혈액 검사와 복부 CT 등으로 진단한다. 식염수 억제검사, 외래 소금 로딩 검사, 또는 fludrocortisone억제검사 (Saline suppression test, ambulatory salt loading test, or fludrocortisone suppression test )등으로 진단한다. History + symptoms + examination findings + other blood tests such as Aldosterone and Renin tests and abdominal CT. Diagnosis is made by saline suppression test, outpatient salt loading test, or fludrocortisone suppression test (Saline suppression test, ambulatory salt loading test, or fludrocortisone suppression test).
치료 treatments	원인에 따라 치료한다. 알도스테론 분비 선종치료는 수술로 하고 부신 과형성치료는 Spironolactone 또는 Eplerenone 등 약물로 치료할 수 있다 그 외 증상에 따라 대증 치료한다. 또 dexamethasone으로도 필요에 따라 치료한다. • Treat according to the cause. • Treatment of aldosterone-secreting adenoma is performed with surgery. • Adrenal hyperplasia can be treated with drugs such as Spironolactone or Eplerenone. • Treat symptomatically according to other symptoms. • Treat as needed with dexamethasone.
