ASD의 정의

  • 자폐증 스펙트럼 장애는 생물학적면에 근거를 두고 정의한다면유전성 경향이 있고 다음과 같은 증상이 있는 신경발육 장애(Neurodevelopemental disorders)이다.
  • 언어나 문맥의 일부에만 제한되지 않고 광범위하게 사회 의사소통과 사회 상호 작용 장애가 지속적으로 있고 결핍되어 있다.
  • 각종 행동이나 흥미 또는 활동이 제한되어 있고 또 반복성이 있고
  • 이런 증상 징후가 소아기의 초기 즉 2세 이전에 발병하는 것이 보통이고
  • 이런 증상 징후가 있으면 사회적으로직업적으로 그 외 다른 여러 영역에서 삶을 정상적으로 영유할 수 있는 능력 장애가 생길 수 있다.
  • ASD가 있는 아이들에게 언어 장애와 사회적 행동 장애의 정도는 일률적이 아니고 다를 수 있다즉 그런 장애 정도가 아주 심할 수도 있고경우에 따라 아주 경미할 수 있다.
  • 미 DSM-IV 기준에 따라 도착성 발달 장애(Pervasive developmental disorders/PDD)라고 병명을 부쳤다가 미 DSM-V 기준에 따라 자폐증 스펙트럼 장애(Autism spectrum disorder /ASD)로 병명을 다시 부쳤다.

자폐증 스펙트럼 장애의 유병률

  • 최근 미국 68명 출생아들 중 1명꼴
  • 2022년 1월 PEDIATRICS에 의하면 미 소아 54명 중 1명꼴로 생긴다고한다.(1/28/2022 UPDATE)
  • 2002년 1,000명 아이들 중 6.4
  • 2008년 1,000명 아이들 중 11.3남아 54명 중 1여아 254명 중 1
  • 2011년 1,000명 아이들 중 20
  • 남아여아의 발병률은 2:1~6.5:1

ASD의 원인

  • 확실한 원인은 모른다.
  • 유전요인과 환경요인으로 생긴다고 믿기도 한다.
  • 쌍태아 중 한 쌍태아가 ASD가 있으면 다른 쌍태아에게 ASD가 유발할 가능성이 10명 중 9명이고,
  • 친 형제 중 한명이 ASD가 있으면 다른 형제들에게 ASD가 생길 가능성은 35배라고 한다.
  • 여린 증후군(Fragile X syndrome)이나 결정성 경화증(tuberous sclerosis)이 있는 아이들에게 ASD가 생길 가능성이 많다.
  • 돌연변이 유전으로 ASD가 생길 수 있다고 믿는다.
  • 먹고 마시는 음식물숨 쉬는 공기치료에 쓰는 약물자궁 내 태아가 엄마의 피를 통해 노출됐던 각종 음식물화학물질 성분 또는 약물에 접촉으로 ASD가 생긴다고 믿기도 한다.
  • 예방접종 백신으로 인해 ASD가 생긴다고 했지만 확실히 증명할 수 없다.
  • 2022년 1월 PEDIATRICS에 의하면 임신부나 초기 영유아기 아기들이 공해 화학  물질에 노출 되거나 내분비 선 장애로 인해  ASD가 생길 수 있다고 한다.

증상 징후

  • 증상 징후는 ASD가 있는 아이에 따라 다르나 일반적으로 다음과 같은 ASD 주 증상 징후가 있다.
  • 사회생활 장애가 있다즉 사회적 의사소통 장애가 있다.
  • 판에 박힌 행동을 반복하기고 하고 즉 상동행동을 하기도 한다.
  • ASD가 있는 아이들은 같은 나이 또래들에 비해 사회적 의사소통 기술 발육이 뒤떨어진다.
  • 대개의 경우 부모들은 ASD가 있는 자녀의 행동이 비정상적인 것을 인지할 수 있다.
  • 심하게는 영유아기 초기부터 발육이 비정상적이고 한 개의 물체에 과도히 관심을 둔다눈길접촉(눈맟춤)이 빈약하거나 없고 피카부(까꿍까꿍등 놀이를 할 수 없다상호작용이 미약하고 부모와 같이 놀 때도 종알종알 아기 소리를 내지 않는다.
  • 일부의 ASD를 가진 아이들은 2세나 3세 될 때 자기 이외 것에 흥미를 잃고 조용하고 위축되고 그 주위 사회에서 일어나는 일들에 무관심하고 위축되기도 한다.
  • 사회적 장애가 심하게 생기고
  • 그들의 주위에 있는 사람들에게 무관심하거나 귀를 기울이려하지 않고 다른 사람들에게 반응을 하지 않는다.
  • 자기들이 좋아하는 장난감이나 행동을 즐거이 혼자하면서 다른 사람들과 서로 같이 즐겨하려 하지 않는다그리고 다른 사람에게 자기가 하는 것을 보이려고 하지 않는다그러나 다른 사람들이 자기가 하고 있는 그런 것을 보면 화내고 불안하고 고통스러워한다.
  • 최근 연구에 의하면 상호 의사소통을 할 때상대자의 눈길을 접촉하는 대신 입을 쳐다보는 경향이 있다고 한다정상적으로 1세가 되기까지 ASD가 없는 아이들은 적어도 한 두 단어를 써서 말 할 수 있고 그의 이름을 부르면 그 소리가 나는 쪽으로 고개를 돌려 쳐다보는 것이 보통이다가지고 싶은 장난감을 보고 달라고 가리킬 수 있다가지고 싶지 않은 경우는 확실히 말로 몸짓 말로 얼굴 표정으로 아니라고 표현 할 수 있는 것이 정상이다.
  • ASD가 있는 아이는 자기 이름을 불러도 반응하지 않거나 천천히 반응하고 말을 걸어도 주의를 기울이지 않는다.
  • 다른 사람들에게 자기가 가지고 놀고 있는 무엇을 보이려고 하지도 않는다.
  • 돌 전 영아를 엄마가 안고 사랑하고 조용히 속삭이면 종알종알 지껄이는 것을 볼 수 있지만 ASD가 있는 영유아들은 그런 종알종알 지껄이는 말도 하지 않고 전반적으로 언어 발육이 지연되고 그림 등으로 의사소통을 하거나 한 개 단어로 의사 소통을 하기도 한다단 한 개 단어로 말을 하거나 구절을 여러 번 반복해 말을 하기도 한다.
  • 자기가 들은 단어나 구절을 반복하기도 한다즉 반향언으로 의사소통을 한다든지 자기 자신만 아는 이상한 말을 해서 의사소통을 하기도 한다
  • 반복적이고 상동적인 행동을 할 수 있다이런 증상이 아주 경미 할 수 있고 아주 심해 쉽게 언어 장애가 있는 지 알아볼 수 있다.
  • 어깨 등을 손바닥으로 계속 반복 치기도 하고 이상한 모습으로 걷기도 한다.
  • ASD가 있는 아이들은 장난감 바퀴나 차 등에 달린 움직이는 물체에 환상적이고 그것을 좋아하고 그런 장남감 등을 길게 늘어놓고 놀기도 한다.
  • 다른 사람이 그렇게 길게 놓은 장난감을 움직이면 골나고 싫어한다.
  • 청소기전화기숫자튜브 등에 특별히 강박 관심을 갖기도 한다.
  • 매일 같은 음식물을 먹기도 하고
  • 변함이 없이 하루 종일 같은 행동을 반복하기도 한다그런 루틴을 부모가 바꾸면 화를 낸다.

자폐증 장애 진단 기준 Autistic Disorder Diagnostic Criteria(DSM_IV)

범주-3범주 중 6개나 그 이상

1. 범주-1

사회적 상호작용 장애

(2개나 그 이상이 나타난다)

  • 다양한 비언어적 몸짓 의사소통 장애가 심각하다(눈길 접촉안면 표현몸자세몸짓 등은 사회적 상호작용을 하는데 필요하다.)
  • 또래관계를 적절히 맺는데 실패한다.
  • 즐거움흥미성취감을 자발적으로 다른 사람들과 더불어 나누어 가지려고 하지 않는다.
  • 사회적 문제나 즐거움과 기쁨 등 감정을 서로 나누어 갖지 못한다.

2. 범주 –2

의사소통 장애

(한 개나 그 이상이 나타난다.)

  • 구두 언어 발육이 지연되거나 전적으로 발달되지 않는다.
  • 언어 발육이 있을 경우에는의사소통 시작 능력 장애가 있고의사소통을 지속 능력 장애가 있다.
  • 보통 언어나 특수 언어를 반복적으로 또는 상동적으로 사용한다.
  • 발육수준에 부적절하고다양하고 자발적인 거짓장난을 한다또는 사회적 모방적 놀이를 한다.

3. 범주-3

행동흥미활동이 제한되어 있고 반복적이고 상동적인 패턴으로 나타난다.

(한 개나 그 이상이 나타난다)

  • 한 가지나 또는 그 이상 흥미로운 행동을 상동적으로 또는 제한적으로 한다그 강도나 초점이 비정상이다.
  • 특정비 기능적 통상 또는 의도적 일에 불굴 집착한다.
  • 물건의 한 부분에 지속적으로 열중한다.

4. 발현 나이

3세 이전에 한 개나 그 이상 영역에서 지연 또는 비정상 기능이 나타난다.

5. 제외 기준

임상적으로 일반적 언어지연이 확연히 없을 때

나이에 적절한 자가 도움 기술적응행동의 인지적 발육이 임상적으로 확실히 지연되지 않았을 때

사회적 상호작용 장애

(3가지 항이 있어야 한다

  • 사회적 감정적 상호 간 나누어 갖는데 결함이 있다.
  • 사회적 상호작용에 사용하는 비언어적 의사소통 행동에 결함이 있다.
  • 적절하게 발육단계를 발전하는 데나 유지하는데 결함이 있다.

2. 범주 –2

행동흥미활동의 제한 반복 상동 작용

(2 항이나 그 이상

  • 상동 또는 반복적 말움직이는 운동또는 물체사용
  • 언어나 비언어적 행동의 루틴 또는 의식적인 패턴에 과도히 매달리거나 그런 것을 변화하는 것에 과도히 저항한다.
  • 강도나 초점에서 고도로 제한되고 고정된 비정상적으로 흥미를 갖는다.
  • 감각에 과도로 반응하던지 저하로 반응한다아픔차고 뜨거운 감각이 비정상으로 반응한다냄새에 과도로 민감하고 특별한 소리나 감촉에 이상반응이 생긴다.
  • 빙빙 돌거나 어떠한 불빛에 환상적이다.

3. 발현 나이

증상이 조기 소아기에 나타나고 다룰 수 없을 정도 이상 사회적 요구가 있을 때까지 완전히 증상이 나타나지 않을 수 있다.

4. 장애

증상과 더불어 모든 기능이 제한되고 장애되어 있다.

5. 중증도

1단계 지지가 필요하다.

2단계 상당한 지지가 필요하다.

3단계-2단계보다도 더 지지가 필요하다.

영유아학령기 아이들에게 다음과 같은 증상 징후 중 하나 또는 그 이상 있으면 자폐증

스펙트럼 장애가 있다고 의심할 수 있다.

다음과 같은 증상 징후가 한 개나 그 이상 있으면 자폐증 스펙트럼 장애가 있는지 의심해야 한다.

눈길접촉(눈 맞춤)이 없거나 빈약할 때

부모나 보호자가 가리키는 곳을 보지 않거나 가리키지 못할 때

언어발육이 지연되거나

서로 의사소통을 할 때 비정형 말을 하거나

비정형 말을 반복하거나

상호 의사소통하기가 어렵거나

1인층 명사를 써서 말을 할 수 없거나

양손을 서로 치기도 하고빙빙 돌기도하고 머리를 바닥 등에 반복 치는 운동 등의 상동행동을 하거나

장난감 차바퀴나 그 외 돌아가는 물건불빛 또는 물 등 어떤 물체의 일부분에 대해서 비정상적으로 정신이 빠져있거나

공룡 또는 기후 등에 대해서 강열한 흥미를 갖거나

다른 사람들과 친밀감이 없거나

혼자 놀기를 좋아하거나

특종 소리나 음식물이나 옷 직물에 과도히 반응하거나

어떤 영역에서는 지능이 지연되고 어떤 다른 영역에서는 진보된 기술을 가질 수 있고

그 외

다음과 같은 증상 징후가 있으면 자폐증 스펙트럼 장애가 있을 가능성이 상당히 많다.

생후 12개월 전까지 종알거리지 못하거나 어디를 가리키면 따라서 가리키지 못하거나 가리키는 곳을 보지 않거나

생후 16개월까지 의미있는 단어를 한 개도 써서 말할 수 없거나

생후 24개월까지 단어가 있는 구절을 써서 스스로 말할 수 없거나

어느 연령에서든지 사회적 기술이 없거나

그 동안 하던 말을 하지 못하거나

자폐증 스펙트럼 장애가 각 항에 따른 증상 징후

1. 감각 기관에 다음 증상 징후가 생길 수 있다.

ASD가 있는 아이에게 다음과 같은 감각 증상이 흔히 생긴다.

특정 소음이나 소리에 과도로 예민하기도 하고

자지 이외 다른 사람의 음성에 덜 민감하기도 한다.

아픈 감각에 비정상으로 덜 민감하기도 하고

경미한 촉각에 상당히 예민하게 반응할 수 있다.

2. 주의력 결핍이 있고 활동이 비정상적일 수 있다.

과소로 활동하거나 위축되기도 하고

반대로 과다 활동하기도 한다.

3. 운동 장애가 생길 수 있다.

어설프게 운동할 수 있고

운동 협동 장애가 생길 수 있고

행동곤란이 있을 수 있고

근육긴장 저하증이 있을 수 있다

4. 지능이 낮을 수 있다.

ASD가 있는 아이들의 38%의 IQ는 70이하이고

24%의 IQ는 71~85 정도이고

38%의 IQ는 85이상

5. 언어발육이 늦고 이니 있던 언어발육이퇴행할 수 있다.

ASD가 있는 아이들의 약 25~30%에서 언어발육이 어느 정도 있을 수 있지만 생후 15~24개월 사이 언어 발육이 서서히 퇴행할 수 있다.

손을 흔들거나 손으로 무엇을 가리키는 비언어적 의사소통과 사회성 기술 등이 없거나 빈약할 수 있다.

이런 증상 징후가 서서히 생기기도 하고 갑자기 생기기도 한다.

그런 증상 징후의 정도는 주위 환경에 따라 생기는 스트레스의 영향을 많이 받는다.

6. 대두증이 있을 수 있다

일반적으로 ASD가 있는 아이들의 신생아 때 머리둘레의 크기는 정상이거나 평균보다 적은 것이 일반적이다.

출생 후 첫 1년 동안 점점 머리 둘레가 정상 이상으로 커져 대두증이 생길 수 있다.

ASD가 있는 아이들의 20~30%의 머리 둘레가 정상 이상으로 커져 대두증이 생기나 소아기를 지난 후에는 뇌의 크기가 적어지는 것이 보통이다.

7. 발작을 할 수 있다

12세 이하이고 지능장애가 없는 ASD 아이들에게 뇌전증이 생기는 발병률은 1.8%이다.

12세 이하이고 지능장애가 있는 ASD 아이들에게 뇌전증이 생기는 발병률은 6.1%이다.

12세 이상이고 지능장애가 없고 ASD 아이들에게 뇌전증이 생기는 발병률은 8.9%이다.

12세 이상이고 지능장애가 있는 ASD 아이들에게 뇌전증이 생기는 발병률은 23.7%이다.

8. 위장관 증상이 생길 수 있다.

변비설사복통유분증위식도 역류위장관 염증이 생길 수 있다.

9. 수면 문제가 생길 수 있다.

ASD가 있는 아이들의 40~86%에게 다음가 같은 수면문제가 생길 수 있다.

잠들기 어려움불안밤 수면 중 자주 깰 수 있고

불면증

수면장애 호흡 곤란증

멜라토닌 생성과 조절 문제가 생길 수 있다.

10. 정신 심리 감정 문제가 생길 수 있다.

불안우울증민감초조 등이 생길 수 있다.

11. ASD 가 있는 아이들에게 다음과 같은 병이 동반될 수 있다.

여린 X증후군

결절성 경화증

엔젤맨 증후군

레트 증후군(Rett syndrome)

스미스렘리오피츠 증후군

다운 증후군

차지 증후군(CHARGE syndrome)

신생아 알코올 증후군

PKU 등등

미 소아과 학회의 ASD에 관한 선별검사 권장

다음 사항 중 하나가 있던지 그 이상 있으면 ASD 특별선별 검사를 꼭 받으라고 권장한다.

친 형제자매 중 ASD가 있으면

부모가 ASD가 있나 선별검사를 해달라고 요구할 때

아이를 돌보는 사람이 ASD가 있나 선별 검사를 해달라고 요구할 때

소아과 의사가 ASD가 있나 의심할 때는 ASD 특별선별 검사를 꼭 받으라고 권장한다.

통상적으로 ASD 선별검사를 받으라고 미 소아과 학회는 다음과 같이 권장한다.

생후 18~24개월 사이에 모든 유아들은 정기 건강검진을 받을 때 ASD 특별선별 검사를 통상적으로 받으라고 권장한다.

미국 신경외과 의사 협회와 신경외과의사 위원회의 ASD 두 단계 선별검사 권장

다음과 같이 두 단계로 조사하라고 권장한다.

(1).통상 발육 감시비정형적 발육이 있는 아이들 중 자폐증이 있나 또는 다른 문제를 가지고 있는 아이가 자폐증이 있나 알아보기 위해 특별 선별검사를 한다.

(2).자폐증을 진단 평가한다그리고 다른 발육 장애와 감별 진단 한다

자폐증 진단

(1) ASD 진단용 DSM-IV와 DSM-V 기준에 따라 임상적 진찰을 한다

생후 18~24 개월 M-CHAT/CHT,

(3) 병력 신경 검사

출생전후기성장발육퇴행뇌증주의력 결핍 문제경련우울증조급증행동이

불규칙적이고식이 이상수면 문제신경 피부문제구어 언어이상비 구어 언어놀이이상 상태에 관한 병력을 알아본다.

Modified Check Iist for Autism in Toddlers (M-Chart)를 이용해서  자폐증 선별검사를 하면 진단하가가 더 쉽다. Pediatrics. 8/2020 

(4) 다른 병과 감별 진단하기 위해

  • 청력 검사와 중이 진찰을 받는다시력 검사를 받는다.
  • 이미증이 있으면 혈중 납 농도 검사를 한다.
  • 여린 X증후군레트 증후군(Rett syndrome) 등이 있으면 염색체 검사를 한다.
  • 대사장애가 있는 것 같으면 대사장애 검사를 한다.
  • 퇴행경련경련을 의심하면 EEG 검사를 한다.
  • 신경 영상 검사는 통상적으로 권장 않는다.
  • 갑상선 기능 검사 등 혈액검사는 통상적으로 권장하지 않는다.

치료

  • 조기 진단을 해서 조기에 적절한 치료를 시작해서 그 아이가 스스로 자립할 수 있는 기능을 증진시키고 삶의 능력을 발전시키는 것이 주목적이다.
  • 여러 가지 치료방법이 있지만 특별한 치료를 받아야 하는 것이 보통이다.
  • 그와 동시에 그들의 부모들을 도와주어야 한다.
  • 의사소통사회적응행동교정사회적응교육 등을 각 면에 증진 개선시키는데 중점을 둔다.
  • 부모교육정서적지지질병특성지지 제공
  • 자녀를 양육 방식 장기적 교육 계육 수립
  • 특수교육,
  • 언어치료,
  • 인지치료,
  • 사회성기술 훈련
  • 운동치료,
  • 시력치료청력소리촉각 등에 관한 감각을 탐지하는 직업치료와 감각 통합치료,
  • 음악치료
  • 자해 위험 방지
  • 행동개선 훈련과 관리긍정적 행동은 강화시키고 자신이 스스로 생활하고 돕고 사회에 적응할 수 있는 사회기술을 훈련시킨다그러기 위해서 올바른 행동과 의사소통 잘 하는 법을 증진시킨다즉 행동조절 치료
  • 핵심 증상 중증도 완화,
  • 운동기술과 운동 조화 결핍을 보강시키기 위해 물리치료
  • 의사소통 치료
  • 약물 치료
  • 공격적이거나 자해과민하면 Clonidine 등으로 치료할 수 있고
  • 불안하면 buspirone 등으로 치료할 수 있고
  • 과다활동충동적, ADHD 이면 신경 자극제 등으로 치료할 수 있고
  • 수면 문제는 규칙적 수면습관으로 치료하고 멜라토닌 등으로 치료할 수 있다.
  • 대체치료도 할 수 있지만 그 치료 방법이 어디에서 어떤 근거에서 나왔는지 확실히 알아보고 그건 치료를 이용해야 한다.

• Autism Spectrum Disorder, defined on a biological basis, is a neurodevelopmental disorder with a hereditary tendency and symptoms such as:

• Persistent and deficient deficits in social communication and social interaction pervasive, not limited to a subset of language or context.

• A variety of behaviors, interests, or activities are limited and repetitive • The onset of these symptoms is usually early in childhood, before the age of 2 years. • The presence of these symptoms may impair the ability to lead a normal life, socially, professionally, and in many other areas.

임신부나 초기 영유아기 아기들이 공해 화학  물질에 노출 되거나 내분비 선 장애로 인해  ASD가 생길 수 있다고 한다.

According to PEDIATRICS in January 2022, it occurs in 1 out of 54 children in the US, ASD may occur in pregnant women or early infants and babies due to exposure to polluting chemicals or damage to the endocrine glands
