퓨버티 지연(사춘기 지연), Delayed puberty(Delayed Adolescence)
  • 사춘기는 시상하부-뇌하수체-생식샘(선) 축(HPG Axis)이 정상적으로 작용해야 제때에 오고 정상적으로 진행된다.
  • 생식샘 자극 호르몬 분비호르몬(Gonadotropin-releasing hormone/GnRH)은 사춘기 되기 전에 분비되지 않다가 사춘기가 되기 시작하면서 GnRH가 분비되기 시작하고 GnRH는 황체형성 호르몬(Luteinizing hormone/LH)과 난포 자극호르몬(Follicle-stimulating hormone/FSH)분비를 자극하고, 이 두 호르몬은 생식선과 성 스테로이드 호르몬 생선을 자극해서 사춘기가 진생 된다.
  • 그러나 어떤 남아에게는 10세에 사춘기가 시작되고 어떤 남아에게는 14세 시작하는 지 확실히 모른다.
  • 여아가 14세가 될 때까지, 젖가슴이 조금도 발육되지 않는 등 2차 성징이 전혀 나타나지 않거나 유방발육 시작에서부터 성징 발생이 시작하는 기간이 5년 이상 걸리면 여아 사춘기 지연이라고 한다.
  • 이차 성징이 나타났지만 16세가 될 때까지 초경을 하지 않으면 원발성 무월경이라고 한다.
  • 남아들의 경우, 14세까지 고환이 커지지 않거나, 외음기가 5년 이상 걸려서 발육하면 남아 사춘기 지연이 있다고 정의하기도 한다. 
  • 사춘기 지연은 여아들보다 남아들에게 더 잘 생기고 이환율은 2.5%이다. 

사춘기 지연의 원인

  1. 사춘기 지연의 원인은 타고난 체질에 따라
  2. 고나도트로핀(Gonadotropin/성선(샘)자극 호르몬) 결핍증(Hypogonadotropic hypogonadism)- 체질 지연, 중추 고나도트로핀 결핍증, 중추신경 종양, 뇌하수체 기능저하증 등
  3. 고나도트로핀 호르몬 과잉증(Hypergonadotropic hypogonadism)-1차 고환부전증, 터너증후군, 크란펠터증후군, 종양, 안드로젠 감소성, 화학물질 치료 등이 있다. 
  • 사춘기 지연의 0.6%는 동년배들의 성장 발육보다 선천성으로 성장 발육이 늦어 사춘기 지발이 생긴다. 
  • 체질성 사춘기 지발(특발성 사춘기 지발/Constitutional puberty delay of growth and maturation)로 퓨버티 지연과 사춘기가 지발이 생길 수 있다. 
  • 난소 기능 저하 여아 사춘기 지발이 생길 수 있고 
  • 고환 기능이 비정상적으로 저하될 때, 
  • 난소가 없으면 여아 사춘기 지발이 생길 수 있고 
  • 고환이 없으면 남아 사춘기 지연이 생길 수 있다. 
  • 성선 자극성 성선 기능 저하증(저고나도트로핀성 저성선증/저 생식샘 자극 호르몬 생식샘 저하증(Hypogonadotropic hypogonadism)으로 사춘기 지연이 생길 수 있다. 
  • 다음과 같은 경우, 성선 자극성 성선 기능저하증이 생길 수 있고 그로 인해 사춘기 지연이 생길 수 있다. 

중추 신경계 이상으로 생기는 사춘기 지연

  • 뇌 기형 
  • 뇌염이나 뇌막염 등으로 인한 뇌 손상
  • 뇌종양
  • 조직구증 
  • 중추 신경 감염병을 앓은 후 
  • 중추신경 방사선 치료를 받은 후 
  • 뇌 발육 이상 

 고나도트로핀 단독 결핍증(성선 자극 호르몬 단독 결핍증)으로 생기는 사춘기 지연

  • 칼만 증후군(Kallmann syndrome) 
  • 선천성 부신 발육 부전
  • 황체 형성 호르몬(LH)과 난포자극 호르몬의 결핍 

다수 뇌하수체 호르몬 결핍

그 외 원인

  • 정신 질환이 있을 때 
  • 영양실조에 걸렸을 때 
  • 신장·폐·심장, 또는 다른 주요 기관에 만성 질환이 있을 때 
  • 신경성 식욕부진 
  • 여아 운동선수에 생길 수 있는 무 월경 
  • 정신병으로 생기는 무 월경

 과고나도트로핀성 생식선 저하증 Hypergonadotropic hypogonadism

  • 터너 증후군이나 다운 증후군 등 염색체 이상이 있을 때 
  • 누난 증후군 
  • XX와 XY 생식선 장애 
  • 자가면역성 난소염 
  • 화학요법이나 방사선 요법 
  • 다낭 난소증 
  • 그 외 

사춘기가 늦게 오는(사춘기 지연) 원인 중 가장 흔한 원인

  • 성장 발육이 타고난 체질에 의해 지연되는 경우이다. 
  • 타고난 체질에 의해 사춘기가 늦게 오는 아이들의 친 부모 형제자매들 대부분도 사춘기가 타고난 체질에 의해 늦게 왔다는 가족 병력이 있다. 
  • 그들의 성장 발육이 동년배들보다 지연되었었다는 가족 병력이 있는 것이 보통이다. 
  • 다시 말하면 친 형제 자매들의 사춘기가 타고난 체질에 의해 늦게 왔다는 병력이 실제로 흔하다.

사춘기 지연의 증상 징후

  • 신체 기질적 이상이 없이 다만 타고난 체질에 의해 사춘기 지연이 오는 아이들의 성장 발육을 살펴보면 다음과 같다. 
  • 일반적으로 태어난 이후부터 사춘기가 시작되는 나이까지 동년배들에 비해 신장과 체중이 더 작다. 
  • 유방의 성장 발육도 동년배들에 비해 늦게 시작한다. 
  • 사춘기가 됐을 때 정상적으로 오는 음성의 변화도 더 늦게 나타난다. 
  • 병이 없이 사춘기가 타고난 체질에 의해 늦게 오는 아이들은 동년배들에 비해 몸집이 더 작은 편이고 전반적으로 2차 성징 발육이 더 늦게 온다. 
  • 그 때문에 일반적으로 대부분은 마음속으로 은근히 많이 걱정한다. 
  • 그렇지만 그들에게 2차 성징이 일단 나타나기 시작하면 그 후 또래들과 같이 정상적으로 성장 발육되고 2차 성징이 정상적으로 온다. 
  • 타고난 체질에 의한 사춘기지연 이외 다른 원인으로 사춘기가 늦게 올 때의 증상 징후는 여기서 생략하기로 한다. p.00 초경이 늦을 때 참조. 

사춘기 지연의 진단

  • 과거 병력과 가족 병력, 증상 징후, 진찰소견 등을 종합해 사춘기 지연이 있다고 의심되면 임상 검사, 골 연령 검사 등으로 진단한다. 
  • 다시 설명하면, 이유를 막론하고 사춘기가 늦게 오면, 초진에서 세밀한 신체검진을 받는다. 
  • 필요에 따라 피, 대소변, 간 기능, 갑상선 호르몬, 성장 호르몬, 염색체, 뇌하수체 기능 저하증 검사, LH, FSH, GnRH 자극 등을 검사하고 
  • 체질적으로 사춘기 지연이 생기는지, 타고난 체질로 사춘기 지연이 생겼는지 알아야 한다. 
  • 신체 기질적 이상이 없는데 타고난 체질로 사춘기가 늦게 오는 아이들의 대부분은 신체가 전체적으로 균형 있게 자라는 것이 보통이다. 
  • 그러나 동년배들보다 단순히 작은 것이 보통이고 사춘기 지연만이 있는 것이 보통이다. 
  • 사춘기 지연이 있는 남아들의 고환은 그렇지 않은 동년배들의 고환보다 더 작고, 
  • 여아들의 유방은 그렇지 않은 동년배들의 유방의 크기나 발육에 비해 더 작은 것이 보통이다. 
  • 음모가 날 때가 훨씬 지났는데도 나지 않을 수 있다. 
  • 사춘기 때 친 부모 형제자매들의 2차 성징이 그들의 동년배들에 비해 늦게 왔고 
  • 사춘기가 늦게 왔었다는 가족 병력이 있는 아이들 역시 사춘기가 선천적으로 늦게 오는 경우가 많다.
사춘기 지연의 치료 
  • 타고난 체질이나 그 외 다른 원인으로 사춘기가 늦게 오는지, 어떤 병으로 사춘기가 늦게 오는지 원인에 따라 치료 방법이 다르다. 
  • 타고난 체질로 사춘기가 지연 될 때는 2차 성징이 확실히 시작될 때까지 얼마 동안 주기적으로 세밀히 재검진을 받고 그때마다 왜 사춘기가 지연되는지 다시 추적 평가를 받는다. 
  • 타고난 체질로 사춘기가 늦게 오고 신체에 이상이 없을 때는 동년배들과 같이 사춘기가 때가 되면 올 것이라고 안심시킨다.
  • 얼마 동안 기다리는 것 이외 별다른 치료방법이 없다고 확신시킨다. 
  • 어떤 원인으로 사춘기가 늦게 오는 경우와 타고난 체질에 의해 사춘기가 늦게 오는 경우를 감별 진단하기 위해 적어도 6~12개월마다 추적 신체검진을 받고 필요에 따라 적절한 임상 검사를 다시 받고 다시 재평가를 받아야한다. 
  • 사춘기가 늦게 오는 특별한 이유가 없이 타고난 체질로 사춘기가 늦게 온다고 진단을 받았을 때는 2차 성징이 나타날 때까지 6~12개월마다 반복 추적 검진을 받고 관찰 치료를 받는다. 
  • 관찰적 치료를 받는 중 2차 성징이 정상적으로 나타나지 않고 2차 성징이 제때에 나타나지 않을 때는 적절한 피검사와 신체검사 등을 반복한다. 
  • 신체적 이상이 없이 사춘기가 타고난 체질로 늦게 와서 많이 고민하는 여아들의 경우, 소량의 에스트라디올 여성 호르몬으로, 남아들의 경우, 소량의 테스토스테론 남성 호르몬으로 치료하는 경우도 있다. 
  • 뇌에 생긴 종양이나 감염병, 또는 선천성 기형, 영양실조, 칼만 증후군, 염색체 이상 등으로 생긴 사춘기 지연은 그 원인에 따라 치료한다.

Delayed puberty (Delayed Adolescence)

• Puberty comes in time and proceeds normally only when the hypothalamus-pituitary-gonadal (gland) axis (HPG Axis) works normally.
• Gonadotropin-releasing hormone (GnRH) is not secreted before puberty, but GnRH begins to be secreted around puberty. -stimulating hormone/FSH) secretion, these two hormones stimulate the gonads and sex steroid hormone fish, which leads to puberty.
• However, it is not clear whether puberty begins at age 10 in some boys and begins at age 14 in others.
• By the time a girl turns 14, no secondary sexual characteristics, such as no breast development, or the onset of sexual characteristics from the beginning of breast development to the onset of sexual characteristics takes more than 5 years, it is called delayed puberty in girls.
• If secondary sexual characteristics develop but menstruation does not occur by the age of 16, it is called primary amenorrhea.
• In the case of boys, it is sometimes defined as delayed puberty in boys when the testes do not grow until the age of 14 or when the vulva takes more than 5 years to develop.
• Delayed puberty is more common in boys than in girls, with a morbidity rate of 2.5%.
Causes of delayed puberty
1. The cause of puberty delay depends on the innate constitution
2. Gonadotropin (gonadotropin) deficiency (hypogonadotropic hypogonadism)- constitutional delay, central gonadotropin deficiency, central nerve tumor, hypopituitarism, etc.
3. Hypergonadotropic hypogonadism – Primary testicular insufficiency, Turner’s syndrome, Cranfelter’s syndrome, tumor, androgen decrease, chemical treatment, etc.
• 0.6% of the delayed puberty is congenital than the growth and development of the same age, and delayed puberty occurs.
• Constitutional puberty delay of growth and maturation can cause puberty delay and puberty delay.
• Girls with reduced ovarian function may develop delayed puberty and
• When testicular function is abnormally low,
• Absence of ovaries can lead to delayed puberty in girls and
• Absence of testes can lead to delayed puberty in boys.
• Gonadotropic hypogonadism (hypogonadotropic hypogonadism/hypogonadotropic hypogonadism) may lead to delayed puberty.
• Gonadotropinic hypogonadism may occur if:
Delayed puberty due to central nervous system abnormalities
• Brain malformations
• Brain damage due to encephalitis or meningitis
• Brain Tumors
• Histiocytosis
• After suffering from a central nervous system infection
• After receiving central nerve radiation therapy
• Brain Dysplasia
Delayed puberty due to gonadotropin monodeficiency (gonadotropin monodeficiency)
• Kallmann syndrome
• Congenital adrenal insufficiency
• Luteinizing hormone (LH) and follicle-stimulating hormone deficiency
Multiple pituitary hormone deficiency
other causes
• When you have a mental illness
• When you are malnourished
• When you have chronic diseases of your kidneys, lungs, heart, or other major organs
• Anorexia nervosa
• Amenorrhea that can occur in female athletes
• Menstrual amenorrhea caused by mental illness
Hypergonadotropic hypogonadism
• When there is a chromosomal abnormality such as Turner’s syndrome or Down’s syndrome
• Noonan syndrome
• XX and XY gonad disorders
• Autoimmune oophoritis
• Chemotherapy or radiation therapy
• polycystic ovary
• etc
The most common cause of late puberty (delayed puberty)
• Growth and development is delayed due to innate constitution.
• Most of the biological parents of children whose puberty is delayed due to their innate constitution also have a family history of delayed puberty due to their innate constitution.
• It is common to have a family history of delays in their growth and development than their peers.
• In other words, the history of late puberty among siblings due to their innate constitution is actually quite common.
Symptoms, signs of delayed puberty
• The growth and development of children with puberty delay due to their innate constitution without physical abnormalities are as follows.
• In general, from birth to the onset of puberty, they are shorter in height and weight than their peers.
• Breast growth and development also start later than in the same age group.
• In puberty, the normal voice changes appear later.
• Children who arrive late due to their innate constitution without disease are smaller than their peers and generally develop secondary sexual characteristics later.
• That’s why most people usually worry a lot in their minds.
• However, once secondary sexual characteristics begin to appear in them, they grow and develop normally like their peers, and secondary sexual characteristics come to normal.
• Symptoms and signs of late puberty due to causes other than puberty delay due to innate constitution will be omitted here. See p.00 when menarche is late.
Diagnosis of delayed puberty
• If it is suspected that there is delayed puberty by synthesizing past medical history, family history, symptom signs, and examination findings, a clinical examination and bone age test are used to diagnose.
• In other words, if puberty comes late for any reason, a detailed physical examination is performed at the first visit.
• Blood, urine and urine, liver function, thyroid hormone, growth hormone, chromosome, hypopituitary test, LH, FSH, GnRH stimulation, etc. are tested as needed.
• It is necessary to know whether puberty delay is caused by constitutional constitution, or whether puberty delay is caused by innate constitution.
• Most of the children who come to puberty late due to their innate constitution even though there are no physical abnormalities, it is normal for their body to grow in a balanced way.
• However, they are usually simply smaller than their peers and have only delayed puberty.
• The testes of boys with delayed puberty are smaller than those of their peers without
• Girls’ breasts are usually smaller than their peers’ breast size and development.
• Conspiracies may not appear long after they are born. • During puberty, the secondary sexual characteristics of biological parents and siblings came later than their peers.
• Children with a family history of late puberty are also often born with late puberty.
Treatment of delayed puberty
• Treatment methods differ depending on the cause, whether puberty is delayed due to innate constitution or other causes, or a disease that causes puberty to arrive late.
• When puberty is delayed due to innate constitution, a detailed re-examination is performed periodically for some time until secondary sexual characteristics start clearly, and each time, follow-up evaluation is performed again to determine why puberty is delayed.
• Reassurance that puberty comes late due to innate constitution, and when there is no abnormality in the body, puberty will come when it is time to go to puberty with the same age group.
• Convince them that there is no treatment other than waiting for a while.
• In order to differentiate between cases of delayed puberty due to any cause and cases of delayed puberty due to innate constitution, follow-up physical examinations at least every 6 to 12 months, appropriate clinical examinations, and reevaluation should be performed if necessary.
• If puberty is delayed for no particular reason, if you are diagnosed as having late puberty due to your innate constitution, repeat follow-up check-ups every 6 to 12 months and receive observational treatment until secondary sexual characteristics appear.
• During observational treatment, if secondary sexual characteristics do not appear normally and secondary sexual characteristics do not appear in time, appropriate blood tests and physical examinations are repeated.
• Girls who suffer from late puberty without physical abnormalities are sometimes treated with a small amount of estradiol, and for boys, with a small amount of testosterone and male hormone.
• Delayed puberty caused by brain tumors or infectious diseases, congenital anomalies, malnutrition, Kalman syndrome, or chromosome abnormalities, is treated according to the cause.
