22q11.2 Microdeletion Syndrome 22q11.2 미세 유전자 결손 증후군,(DiGeorge syndrome and velocardiofacial syndrome, Phelan/McDermid Syndrome)

원인

• 22q11.2 미세 유전자 결손으로 생기는 염색체 이상 질환이다.
• 즉 상염색체 22에 있는 q11.2 미세 유전자 결손으로 이 병이 생긴다.
• VCFS (velocardiofacial syndrome)의 유병률은 5000 출생아들 중 1명 꼴이고
• DiGeorge syndrome의 유병률은 2000~4,000 출생아들 중 1명꼴이다.
• 이 병을 다음과 같이 여러 다른 병으로 부르기도 한다.
• autosomal dominant Opitz G/BBB syndrome,
• CATCH22 Cayler cardiofacial syndrome,
• donotruncal anomaly face syndrome (CTAF)
• deletion 22q11.2 syndrome
• DiGeorge syndrome
• 22q11.2DS Sedlackova syndrome
• Shprintzen syndrome
• VCFS (velocardiofacial syndrome)
• velo-cardio-facial syndrome.

증상 징후

• 구개파열, 입천장인두기능부전, 활로 4증, 심실중격결손증, 안면기형, 부갑상선 저하증과 저캴슘증, 면역결핍증, 식이곤란, 신장기형, 청력상실, 후두기관식도기형, 성장호르몬 결핍, 자가면역 이상, 뇌전증, 골격이상, 학습장애, 저능아, 정신질환, 정신분열증, 흉선 발육부전, T 세포 면역결핍증, 과다 활동장애, 우울증 등의 증상 징후, 기형이 생길 수 있다.
• 특히 DiGeorge 증후군이 있을 때는 흉선형성저하증이 있으며 신생아 저칼슘증이 생기고 식이곤란이 생기고, 빨아먹는 힘이 약하고 삼키는데도 힘이 약하다. T세포 면역결핍증,
• 저 개가 낮게 위치하고, 양미간이 넓고, 위입술이 가늘고, 학습장애, 근육긴장저하 등 증상 징후 기형이 생길 수 있다.

진단

병력 증상 징후 진찰소견을 종합해서 이 병을 의심하면 FISH 염색체 핵형분석으로 진단한다.

치료

종상 증후 기형의 정도에 따라서 치료한다.

심장, 정신, 신경, 내분비, 성장 발육 등 소아 특수전문의들이 상호 협력하에 치료한다.

cause

• 22q11.2 It is a chromosomal abnormality caused by microgene deletion.

• The disease is caused by a q11.2 microgene deletion on autosomal 22.

• The prevalence of velocardiofacial syndrome (VCFS) is 1 in 5000 live births.

• The prevalence of DiGeorge syndrome is 1 in 2000-4,000 births.

• This disease is sometimes called by several other diseases, such as:

• autosomal dominant Opitz G/BBB syndrome;

• CATCH22 Cayler cardiofacial syndrome;

• donotruncal anomaly face syndrome (CTAF)

• deletion 22q11.2 syndrome

• DiGeorge syndrome

• 22q11.2DS Sedlackova syndrome

• Shprintzen syndrome • VCFS (velocardiofacial syndrome)

• velo-cardio-facial syndrome.

Symptoms Signs

• Palatal rupture, oropharyngeal insufficiency, tracheostomy 4 syndrome, ventricular septal defect, facial malformation, hypoparathyroidism and hypocysemia, immunodeficiency, dyspepsia, kidney malformation, hearing loss, pharyngoesophageal malformation, growth hormone deficiency, autoimmunity Abnormalities, epilepsy, skeletal abnormalities, learning disabilities, children with disabilities, mental disorders, schizophrenia, thymus hypoplasia, T-cell immunodeficiency, hyperactivity disorder, depression, and other symptomatic signs and deformities may occur.

• In particular, when DiGeorge syndrome is present, hypothermia is present, neonatal hypocalcemia occurs, eating difficulties occur, sucking power is weak, and swallowing power is weak. T-cell immunodeficiency,

• The dog is positioned low, the forehead is wide, the upper lip is thin, and symptoms such as learning difficulties and hypotonia may occur.

Diagnosis

If the disease is suspected by combining the medical history, symptoms, symptoms, and examination findings, the diagnosis is made by FISH chromosome karyotyping.

cure

Treatment depends on the severity of the tumor symptom deformity. Pediatric specialists in cardiac, psychiatric, neurological, endocrine, and growth development work in cooperation with each other.
