Wolf-Hirschhorn syndrome, 울프–헐쉬혼 증후군

원인

  • 염색체 4p(4p16.3) 상실로 생기는 유전증후군이다.
  • 이 병을 4p 증후군, 1염색체 4p, chromosome 4p deletion syndrome
  • chromosome 4p monosomy, del(4p) syndrome, 4p deletion syndrome
  • partial monosomy 4p, 4p- syndrome 또는 WHS라고도 한다.
  • 50,000명 출생아들 중 1명꼴로 유발한다.
  • 여아들에게 2배 이상 더 생긴다.
  • 이 증후군은 피트–로저스–댕크스 증후군(Pitt-Rogers-Danks syndrome)과 거의 같은 증후군이다.

증상 징후

  • 이 증후군을 가진 아이의 얼굴은 헬멧을 쓴 그리스 용사의 얼굴모양과 거의 같다.
  • 앞이마가 불쑥 나오고 미간이 두드러지게 툭 나오고
  • 코가 넓고 새부리처럼 생겼고
  • 눈알이 툭 나오고 내안 각 주름이 생겨있고
  • 양쪽 구각이 축 늘어지고 구개파열이나 토순이 생겨 있을 수 있고
  • 턱이 작고
  • 심장기형, 요로기형, 생식기기형 등 선천성 기형이 있을 수 있고
  • 내반첨족, 척주측만곡증, 척주 후만증, 근력저하,
  • 성장발육 지연, 저신장증,
  • 건성피부,
  • 재발성 중이염, 감각 신경성 청력상실,
  • 중추신경 이상, 소두증, 뇌전증,
  • 지능저하 등 여러 종류의 증상 징후가 생길 수 있다.

진단

병력 증상 징후, 진찰소견 등을 종합해서 이 병을 의심하면 염색체, 핵형 분석으로 확진한다.

치료

  • 심장기형에 관해서는 소아 심장내과 전문의의 치료를 받고 그리고 뇌전증에 관해서는 소아 신경내과의 치료를 받는다.
  • 그외 행동 문제, 사회성 문제, 성장발육 장애, 눈 문제, 골격이상, 청력상실 등의 문제가 있으면
  • 그때그때 증상 징후에 따라 적절히 치료받는다.

Wolf-Hirschhorn syndrome

Cause

• It is a genetic syndrome caused by loss of chromosome 4p (4p16.3).

• This disease is 4p syndrome, chromosome 4p, chromosome 4p deletion syndrome

• chromosome 4p monosomy, del(4p) syndrome, 4p deletion syndrome • Also called partial monosomy 4p, 4p-syndrome or WHS.

• Causes 1 in 50,000 live births.

• More than twice as common in girls. • This syndrome is almost identical to Pitt-Rogers-Danks syndrome.

Symptoms, Signs

• The face of a child with this syndrome is almost identical to that of a Greek warrior wearing a helmet.

• The forehead protrudes and the forehead stands out.

• The nose is broad and like a bird’s beak,

• The eyeballs are protruding and each inner wrinkle is formed.

• Both corners of the mouth are drooping and there may be cleft palate or vomiting.

• Small chin

• There may be congenital malformations such as heart malformations, urinary tract malformations, and reproductive malformations.

• varus cusp, scoliosis, kyphosis, muscle weakness,

• Growth retardation, short stature,

• dry skin,

• recurrent otitis media, sensorineural hearing loss,

• CNS abnormalities, microcephaly, epilepsy,

• Several types of symptoms, such as intellectual decline, may occur.

Diagnosis

If the disease is suspected by synthesizing the medical history, symptoms, signs, and examination findings, the diagnosis is confirmed by chromosome and karyotype analysis.

Treatment

• For cardiac anomalies, see a pediatric cardiologist, and for epilepsy, see a pediatric neurologist.

• If you have other problems such as behavioral problems, social problems, growth and development disorders, eye problems, skeletal abnormalities, hearing loss, etc.

• Receive appropriate treatment according to the symptoms and signs.
